Intermittent Fasting: What the Evidence Actually Shows
Yes, intermittent fasting works for weight loss, and the trial evidence is clear about why: it lowers how much you eat. When researchers pooled 99 randomised trials in 2025, every fasting style beat eating freely, and against ordinary calorie cutting the differences were small or nil.1 That is not a debunking. A clock that helps you eat less, without counting anything, is a useful tool, and for some routines it is the best tool available. The calorie deficit appears to do most of the work. Meal timing may add a small effect in some settings, particularly earlier eating windows.2 But trials have shown nothing resembling the metabolic advantage fasting is marketed as having, and that marketing sets people up to fast harder when the real problem lies elsewhere. Intermittent fasting is an eating pattern that limits when you eat, not what you eat, so that total intake falls. Three schedules carry almost all the research. In 16:8, each day holds a sixteen-hour fast and an eight-hour eating window, with 14:10 as its gentler cousin. In 5:2, five days run normally and two drop to roughly 500 to 600 kcal. Alternate-day fasting swings between normal days and ~500 kcal days. One-meal-a-day sits at the aggressive end of the same family.
Notice what none of them mentions. Food. Remember that when you pick a schedule, because a rule about hours can only ever influence what you eat indirectly, through the meals it leaves room for and the ones it quietly removes. A 16:8 window filled with fried snacks and sweet chai can obey every timing rule and still fail to create a calorie deficit. That single observation, that the clock structures eating while the resulting energy intake appears to explain most of the weight change, explains nearly everything the trials below found.

About the same, and the evidence for that answer is now unusually strong. The 2025 BMJ network meta-analysis pooled 99 randomised trials across all three fasting styles. Every style reduced body weight compared with unrestricted eating. Head-to-head against ordinary daily calorie cutting, only alternate-day fasting edged ahead, by about 1.3 kg. The authors graded that as below their own 2 kg threshold for clinical relevance, and it showed mainly in trials shorter than 24 weeks.1 Roughly two-thirds of the 6,582 pooled participants were women, so the headline result is drawn largely from them. A February 2026 Cochrane review reached the same practical conclusion from a narrower set of trials: across 22 studies and 1,995 adults with overweight or obesity, fasting produced little to no additional weight loss compared with conventional dietary advice, though the certainty of that estimate was low.3 The two reviews are not in conflict. They set different entry rules and arrive at the same verdict: fasting is a usable method, never a uniquely superior one.
The individual trials behind that pooled picture tell the same story with faces on. In a one-year randomised trial of alternate-day fasting, the fasting group lost no more weight or fat than daily calorie cutters, and dropout was higher.4 A 2024 meta-analysis similarly found no overall advantage in body weight or fat mass. Intermittent restriction did, however, produce a small additional reduction in fat-free mass and waist circumference, another reason to guard protein and resistance training during any weight loss.5 So the fair verdict runs in both directions at once. Fasting is not superior to calorie cutting. It is also not inferior, and for the person who has tried and abandoned four food diaries, who finds counting miserable and a cut-off liberating, “equally effective and much easier to follow” is a perfectly good reason to choose it.

Mostly the calories, with a possible small contribution from timing. Two elegant trials make the first half of that case. In the TREAT trial, 116 adults were randomised to 16:8 or three meals a day, with no other instructions. The fasting group lost a statistically unremarkable 0.26 kg more. In the smaller body-composition subgroup, lean mass in the arms and legs fell more in the fasting group; protein intake was not measured, so reduced protein is a plausible explanation, never a demonstrated one.6 And in a one-year trial from Guangzhou, everyone cut calories while half also kept an 8 am to 4 pm window; the window added no extra loss beyond the calorie cut itself.7 Those two trials argue that the calorie deficit explains most of the effect. Two newer results keep that conclusion humble. A 2025 meta-analysis of 30 trials and 1,341 participants found that, even when calories were matched, time-restricted eating was associated with 1.46 kg more weight lost and 1.50 kg more fat lost. Fat-free mass fell further too, by 0.41 kg. The authors read the pattern as a possible body-clock effect.8 And the TREAD trial, published in June 2026, shows why the practical value of a window should not be dismissed. Sixty-one adults with obesity were randomised to a personalised 10-hour window or standard counselling. At 12 weeks the window group had lost about 2.9 kg more, mostly fat.9 Calories were not matched, so the trial cannot say whether the clock itself caused the difference, only that the rule helped people lose more in practice. Worth saying plainly: small timing effects are real possibilities, but nothing here supports the large metabolic advantage fasting is often marketed as having.
What about the celebrated extras, autophagy and “metabolic switching”? Real biology, mostly demonstrated in animals and short human studies, and not yet shown to produce better human weight or health outcomes than the same deficit achieved another way. An umbrella review of the meta-analyses found the evidence base broadly supportive on weight and some cardiometabolic markers, and thin on the grander claims.10 One interesting exception deserves careful telling: in a small, tightly controlled study, eight men with prediabetes who ate all meals in an early six-hour window improved insulin sensitivity and blood pressure without losing weight.11 Eight people is a signal, never a verdict. It does point the same direction as the post-meal glucose biology: when you eat may matter somewhat for glucose handling, even while calories decide most of the weight.
Is 16:8 the best intermittent fasting method? Copy link
For many people, 16:8 is the simplest fasting pattern to sustain, and that matters more than small differences between schedules. The 16:8 pattern demands no fasting days, no calorie arithmetic, and usually only a shift or compression of ordinary meals, so it leaves the family dinner intact. Its trial performance is modest, as TREAT showed, precisely because it is gentle. Alternate-day fasting produced the largest average loss in the 2025 network analysis, but its advantage over ordinary calorie restriction was only about 1.3 kg, and there was no clear winner between time-restricted and whole-day fasting.1 So choose on adherence. Not on a league table.
The practical guidance falls out of that trade. Pick 16:8, or the softer 14:10, if your goal is a sustainable structure with a firm cut-off for evening snacking. Pick 5:2 if you would rather concentrate the discomfort into two planned days than dilute it across seven. Whichever you pick, two rules protect the result. First, a normal day means a normal day, never a compensation feast. Second, every eating window needs its protein, planned deliberately, because a short window makes the usual per-kilogram target easy to miss: a four-society advisory recommends between 1.2 and 1.6 grams for every kilogram you weigh during active weight loss, which lands most adults at 80 to 120 g a day.12 The Indian food arithmetic is in the protein guide.

Nobody knows for certain, because the trials are thinner than the confidence around them. Much thinner. Women have taken part in many fasting trials, but most studies were not designed to answer whether women respond differently from men, and female-specific reproductive outcomes remain much less studied. Claims that fasting "wrecks female hormones" rest more on animal data and anecdote than on human trials.
Two groups sit in different boxes, and it helps to keep them apart. Safety first: fasting is not appropriate in pregnancy or while breastfeeding, when energy needs go up, not down. A history of disordered eating is a reason not to start, because rigid food rules can reopen an old door.13 Individualisation second: PCOS and treated hypothyroidism are not automatic contraindications; they are reasons to plan the schedule with your doctor. PCOS guidance recommends lifestyle change without favouring any one diet pattern, and the fasting-specific evidence is early and thin.14 If you take levothyroxine, its dosing conditions need to stay consistent.15 And if cycles turn irregular after an aggressive schedule, treat that as a signal to step down the intensity; pushing through is the one wrong answer.

Yes, better than most imported diet ideas, once you fix two things: the late dinner and the protein. The familiarity is real: between vrat days, Ekadashi, Navratri and Ramzan, structured fasting is already woven through Indian life in a way it simply is not in the countries where most of these trials were run, and a 16:8 window can feel less foreign here than an imported diet usually does. The frictions are real too. The first is that Indian dinner is late and social, often the day's main family event, so the textbook advice to close the window by 7 pm collides with how households actually eat. In practice, the fix is unglamorous: anchor the window around your dinner, say 12 pm to 8 pm, instead of importing an American schedule. As you would expect, an imperfect window you keep beats a perfect one you abandon.
The second friction is what fills the window. The Indian plate already runs carbohydrate-heavy, and the national dietary survey measured it: carbohydrate at 62.3 per cent of energy, protein at just 12.16 Compress that plate into eight hours, same two rotis and same small katori of dal, and fitting enough protein into the remaining meals can become harder, while sweet chai, which almost no window survives, quietly breaks your fast at 11 am. So the Indian version of the rules. Milk chai belongs inside the eating window; outside it, stick to water, plain tea or black coffee. Who should not do intermittent fasting? Copy link
Anyone on insulin or sulfonylureas, anyone pregnant or breastfeeding, anyone with an eating-disorder history, and underweight, teenage or frail older people, none of them without a doctor. The safety lines matter more than the schedule. People taking insulin or sulfonylureas should not rearrange meal timing without medical review. These medicines can increase the risk of hypoglycaemia during fasting, though individual risk also depends on the regimen, glucose control, previous hypoglycaemia and other health conditions, which is why the diabetes-and-Ramadan guidelines assess each person rather than each drug.17 The same doctor-first rule applies in pregnancy and breastfeeding, and to anyone with a history of disordered eating. It applies too to underweight adults, teenagers and frail older people, who have no spare margin to fast away.13

Usually because the calorie deficit inside the schedule has quietly closed, portion by portion. The schedule, which never changed at all, takes the blame. The window itself does not guarantee a calorie deficit, so check its contents before you touch its length: oversized meals inside a compliant window, liquid calories, and portions that crept as the routine got comfortable all erase a deficit while the clock still reads perfect. Can I drink chai or coffee while fasting?
Plain tea or black coffee, yes. Chai made with milk or sugar contains calories and belongs inside the eating window, even if no sugar is added. Which is better: intermittent fasting or a normal calorie-deficit diet?
The trials say they work about equally well, so the better one is the one you can keep for months. Choose fasting if rules and cut-offs suit you better than counting; choose ordinary calorie reduction if skipping meals makes you overeat later or collides with family life.
Does intermittent fasting slow your metabolism?
Ordinary schedules like 16:8 or 5:2 have not been shown to slow metabolism beyond what accompanies any weight loss. Losing weight by any method lowers daily calorie burn, because a smaller body spends less. Protecting protein and strength training limits the muscle component of that decline.
Is intermittent fasting good for people with diabetes?
Sometimes, and only with medical supervision. Weight loss helps blood sugar, and structured eating can too, but fasting on insulin or sulfonylureas without dose adjustment risks hypoglycaemia. Anyone with diabetes should plan any fasting schedule, religious fasts included, with their doctor first.
What breaks a fast?
In most time-restricted-eating studies, the fasting window excludes caloric foods and drinks. Water, plain tea and black coffee are generally permitted; milk, sugar, juice and snacks belong inside the eating window. Medicines should be taken as prescribed regardless of the fasting schedule, and never delayed to protect a window.
Should I try fasting or ask about weight-loss medication?
They answer different situations. A fasting schedule is a reasonable structure for building a calorie deficit.
