TSH Normal Range: Levels by Age, for Women, High and Low
For most adults, a normal TSH sits between about 0.4 and 4.0 mIU/L. Indian labs do not all agree, and some print an upper limit as high as 5.5. Read the range beside your own number before you compare it with anything else. A high TSH usually points to an underactive thyroid, and a low TSH to an overactive one.

What is a normal TSH range? Copy link
A normal TSH for most non-pregnant adults is roughly 0.4 to 4.0 mIU/L. The units are milli-international units per litre, and your report may print them as µIU/mL, which is the same number. Your lab may head the row as the TSH normal value, which means the same printed interval.
If you went looking for the thyroid normal value, for normal thyroid levels or for the thyroid normal range, you have landed in the right place. TSH is the figure your report flags first, and it appears on almost every routine blood panel ordered in India.
TSH is thyroid-stimulating hormone, the signal your pituitary sends to the thyroid. It moves in the opposite direction to thyroid activity. When the thyroid slows down and stops delivering enough hormone, the pituitary notices the shortfall and pushes harder, so TSH climbs. When the thyroid runs fast, the pituitary eases off and TSH falls.
That inversion is why the result confuses people. A number flagged high on your report is pointing at a gland doing too little. TSH also responds to small hormone shifts before free T4 moves at all, which is why it is measured first.1
Why your report's range may not match the one you looked up Copy link
Reference ranges are not handed down by a central authority. Each laboratory sets its own, from the analyser it runs and the reference group it sampled, which is why two accredited Indian labs can print different limits for the same hormone drawn on the same morning.
Dr Lal PathLabs states that a TSH above 5.5 mIU/L suggests an underactive thyroid and below 0.35 mIU/L an overactive one.2 Many other reports use 4.0 or 4.5 as the ceiling. A result of 4.8 is therefore flagged on one report and unflagged on another, with no change in the person.
Assay design adds to this. When four TSH methods were compared at the 4.0 mIU/L level, they differed by around 1 mIU/L, a gap comparable to the effect of age.3 Check the interval printed next to your value before you compare it with anything online.
TSH normal range for female adults Copy link
Most routine laboratory reports print one adult TSH reference interval and apply it to non-pregnant men and women alike. Normal TSH levels for females and for males usually come off the same line of your report. Pregnancy is the major clinical exception.
Several Indian health pages publish a women-only table, commonly giving 0.4 to 2.34 mIU/L up to age 29 against 0.4 to 4.15 for men of the same age. Those figures are research percentiles copied from page to page for years, and they are far tighter than anything your laboratory is likely to print. Population studies do detect real sex differences, so research-derived intervals are not identical for men and women, and a chart built from one cohort will not match the interval your own laboratory validated on its own analyser. The direction of that difference points the other way: reference work stratified by sex puts the upper limit in women level with or a little above the limit in men.4
The thyroid normal range in female adults is usually the same printed interval given to men, so check your own report before you compare it with a women-only chart. What differs by sex is how often the gland fails. An eight-city Indian study of 5,360 adults found 15.86% of women hypothyroid against 5.02% of men. About a third of those found to be hypothyroid had not known it before the study tested them.5 Your odds change with sex. The line on the report does not.
TSH normal range by age Copy link
TSH drifts upward with age. The drift is real, and it is large enough at the top of the age range that applying a single adult ceiling to an eighty-year-old will label some of them abnormal for ageing alone. In the NHANES III population, the 97.5th percentile climbed from 3.56 mIU/L among people aged 20 to 29 to 7.49 mIU/L in those aged 80 and above.6
The practical size of that drift is smaller than the tables suggest. When investigators applied age-specific upper limits instead of a single 4.0 cut-off across 150,000 consecutive TSH results from a Western Australian laboratory, only 0.1% to 1.9% of people in most age bands changed category. Above 85, reclassification reached 2.1% to 4.7%.3
So an age chart changes almost nothing for a 34-year-old and something worth knowing for an 84-year-old. In a much older adult, a mildly raised TSH may simply be what ageing looks like on a thyroid panel, and that is one reason guidelines fold age into the treatment decision alongside symptoms and cardiac history.7 Age shifts the threshold. It does not erase it.
What "TSH ultrasensitive" and "3rd generation" mean on your report Copy link
Both labels describe the lowest TSH the assay can measure reliably. Neither describes a different hormone, a stricter range, or a more serious test.
Assays are grouped by functional sensitivity, which is the level below which the method's imprecision exceeds a coefficient of variation of 20%, so the grouping is a statement about reliability at the bottom of the scale. Second-generation methods reach roughly 0.1 mIU/L and third-generation methods roughly 0.01 mIU/L.8 Each generation buys one more decimal place at the bottom of the scale.
Dr Lal PathLabs lists its standard test as TSH (Thyroid Stimulating Hormone) Ultrasensitive, Serum.9 That is the default product name at India's largest chain. Nobody sold you an upgrade. Extra sensitivity earns its keep when TSH is suppressed near zero, which happens in an overactive thyroid and in people on suppressive doses of thyroid hormone after thyroid surgery or ablation. If your number is high, the word on the report adds nothing to it.
What a high TSH means Copy link
A high TSH usually means the thyroid is underactive, and the size of the number shapes what happens next. Your free T4 decides the rest, and both hormones sit on the standard full body checkup panel.
That second measurement is what separates a thyroid that has already stopped keeping up from one that is still managing, and it is the reason a raised TSH on its own is rarely enough to start a prescription.
When TSH is high and free T4 is low, that is overt hypothyroidism, and treatment is standard. When TSH is high and free T4 is still normal, that is subclinical hypothyroidism, and the threshold applies there. ATA and AACE guidance advises levothyroxine once TSH passes 10 mIU/L, with an individual decision below it, weighing symptoms, TPO antibodies, cardiovascular risk and age.710 NICE goes further and asks for a TSH of 10 or higher on two occasions three months apart before considering treatment.11
Symptoms commonly reported with an underactive thyroid include tiredness, weight gain, cold intolerance, dry skin, constipation and heavier periods, all of which turn up just as often in people whose thyroid is working perfectly well. They overlap heavily with other conditions, so the number and the symptoms are read together. Diffuse hair shedding is among the changes that can follow a sustained rise.
What a low TSH means Copy link
A low TSH usually means the thyroid is overactive and the pituitary has stopped pushing. Free T4 and free T3 are measured next to see how far the picture has gone. A suppressed signal with normal free hormones usually means watching and repeating, while a suppressed signal with raised free hormones is the picture that moves a doctor toward treatment.
A low TSH with raised free T4 or free T3 is overt hyperthyroidism. A low TSH with normal free hormones is subclinical hyperthyroidism, which is often rechecked before anything is decided.
A suppressed TSH also appears in people taking levothyroxine at a dose above their requirement, and in the weeks after an episode of thyroiditis. Recent or current illness can push it down too. Ask whether a repeat is sensible before accepting a single low reading as a diagnosis.
Reading TSH together with free T4 Copy link
TSH alone names the direction. Pairing it with free T4 names the state. This table is the reasoning a clinician applies to your report.

The word subclinical carries the weight here. It means the signal has moved while the hormone itself has not, and that state does not automatically call for a prescription.10
You may have searched the T3, T4 and TSH normal range as one block, by age or for a female relative, and the T3 normal range for female adults is printed the same way as everyone else's. Those extra rows belong with the whole thyroid panel, where each hormone is read against the others.
Is fasting needed for a TSH test? Copy link
No lab guideline requires fasting for TSH. Timing still matters.
The hour you give the sample, and whether you have eaten, can move the result far enough to change which label ends up on your report, which is why two draws taken under different conditions are hard to compare.
TSH peaks overnight and falls through the day, and it drops further after a meal. In an Indian study of 57 ambulatory patients, TSH fell in all of them two hours after food while free T4 held steady. 15 of 20 people classed as subclinical hypothyroid on the fasting sample were inside the range on the fed sample.12 A Chinese randomised study found a fall of roughly 30% two hours after breakfast.13
Both studies are small, both were done in outpatient clinics, and neither followed anyone far enough to show that the reclassified people came to any harm, so treat the finding as a caution about consistency. The useful habit is repetition under matching conditions. If your first sample was an early-morning fasting draw, have the repeat done the same way.
What moves TSH without any thyroid disease Copy link
Several ordinary things shift a TSH result, and most of them are reversible before the next draw.
High-dose biotin is the one worth naming first. Hair and nail supplements commonly contain 5 to 10 mg, against a daily requirement measured in micrograms. Biotin interferes with the streptavidin chemistry many immunoassays use, and the FDA has issued safety communications about it.14
On susceptible biotin–streptavidin immunoassays the pattern is a falsely low TSH alongside falsely high free T4 and free T3. That combination can imitate an overactive thyroid in someone whose gland is fine.15 Susceptibility is platform-dependent, and on some analysers the free hormones are affected while TSH is not.16 If you take a biotin supplement for your hair, tell the lab and stop it for a few days before the draw.
Acute illness and recovery from it can push TSH either way. Steroids, dopamine, amiodarone and lithium all affect thyroid results.
Levothyroxine is slower. Inconsistent use over the preceding weeks is what distorts the picture, and more than half of Indian patients taking levothyroxine sit above their target TSH.17 A tablet taken shortly before the draw behaves differently. It can raise free T4 for several hours, while TSH changes less acutely, because the drug has a half-life of roughly five to nine days and the pituitary is reading a reservoir.18 TSH is not immune to it. In one sampling study, TSH ran about 19% lower in random samples than in trough samples drawn more than 22 hours after the dose.19 Tell your clinician when you last took it.
TSH in pregnancy Copy link
Are you pregnant? Then the ceiling drops. The TSH normal range in pregnancy is lower than the adult one, and the TSH normal range for a female patient in her first trimester is the figure most people ask about. hCG shares enough structure with TSH to stimulate the thyroid receptor directly in early pregnancy, which nudges thyroid hormone up and the pituitary signal down, so TSH falls most noticeably during the first trimester.
The 2026 American Thyroid Association guideline asks for a laboratory- and trimester-specific range wherever one exists. Where none is available, it puts the typical first-trimester interval at about 0.1 to 4.0 mIU/L.20 The older fixed cut-off of 2.5 mIU/L is no longer the default, and pages still quoting it have not been updated.
One detail matters in India. The same guideline says that centres whose non-pregnancy upper limit sits well above 4.5 mIU/L should subtract 0.5 from their own printed limit instead of dropping all the way to 4.0, which is a different instruction from the one most pregnancy pages repeat.20 On a Dr Lal PathLabs report the adult ceiling is 5.5, so that subtraction points to roughly 5.0.
The guideline is candid about how rough these shortcuts are. Measured against laboratory- and trimester-specific intervals, a fixed 4.0 ceiling correctly identifies 46.1% of first-trimester overt hypothyroidism and puts the remainder into the wrong category, calling them euthyroid, subclinical or hypothyroxinaemic instead.20 So ask. Use the trimester range your obstetrician or laboratory gives you.
How to bring a high TSH down Copy link
A high TSH caused by an underactive thyroid comes down with levothyroxine, prescribed and titrated against repeat testing. Nothing else does.
Iodine correction helps only where iodine is actually short, which needs checking before it is assumed. Selenium has been studied in autoimmune thyroiditis with mixed results, and the trials are small. No food, yoga sequence or supplement has been shown to normalise a sustained TSH above the reference range.
Where the rise is mild and free T4 is normal, doing nothing yet is a legitimate plan. A repeat test in six to twelve weeks, sometimes with TPO antibodies to gauge the chance of progression, is often more useful than starting a lifelong tablet on one reading.10
When to repeat the test, and when to see a doctor Copy link
Repeat it first.
A mildly abnormal TSH deserves a second look before anyone treats it. A single value outside the printed range is common, and guidelines build a repeat into the pathway before any diagnosis is made.1011 In older adults, up to 80% of mildly raised TSH results normalise on remeasurement.20
See a doctor promptly for a TSH above 10 mIU/L, for any abnormal result with symptoms, for a suppressed TSH with palpitations or weight loss, and for any thyroid result in pregnancy or while trying to conceive. Bring the actual report. The printed range is half the answer. If the panel included T3 and T4, bring those rows too.
Book the repeat under the same conditions as the first. Same lab. Same time of day, and the same fasting state. Switching laboratory can move a high-normal TSH by about as much as age does.3 That single habit removes most of the noise people mistake for change. Hypothyroidism affected about one adult in ten in the eight-city Indian study, and it is treatable.5

What is a dangerously high TSH level?
No TSH number is an emergency by itself. A high TSH with low free T4 is overt hypothyroidism and is generally treated whatever the number. When free T4 is normal, a persistent TSH of 10 mIU/L or higher is the common threshold for considering levothyroxine, and NICE asks for two readings three months apart.
What is the TSH normal range for females?
Most routine laboratories use the same adult TSH interval for non-pregnant men and women, although population-derived reference intervals can show small sex differences. Indian reports commonly print 0.4 to 4.0 mIU/L, and some print up to 5.5. Pregnancy is the exception and uses a lower ceiling.
Does a TSH test require fasting?

Everyday stress is not an established cause of a persistently raised TSH. Acute illness, recovery from illness, some medicines and high-dose biotin supplements can all shift thyroid results temporarily. An unexpected value is usually repeated before any diagnosis is made.
Is the TSH normal range for newborn babies different?
Yes, and it is far higher. TSH surges in the hours after birth, so newborn screening uses its own cut-offs and its own timing, usually a heel-prick sample taken on day two to day four. Adult figures do not apply. Newborn results are interpreted by the paediatric team.
How can I reduce TSH naturally?
A raised TSH from an underactive thyroid is corrected with levothyroxine under medical supervision. Correcting a genuine iodine shortfall can help where one exists. No diet, supplement or exercise routine has been shown to normalise a sustained high TSH on its own.
