Neutrophils Normal Range: Percentage, ANC, High and Low

Neutrophils are the most numerous white blood cell and the first ones recruited to a site of infection, which is the job a scraped knee asks of them. A blood count catches only the small fraction circulating at that moment, which is why the absolute count, not the percentage, is the number to read.

Neutrophil reference ranges vary between laboratories. Many references give 40 to 60 percent of white blood cells as the normal neutrophil percentage; some Indian laboratories print wider intervals, and an Indian reference study of 10,665 health-check attendees measured 44 to 75 percent in women and 42 to 74 percent in men, against the 40 to 75 percent its own laboratories were already using.12 The absolute neutrophil count, or ANC, is the number of neutrophils per microliter of blood, typically about 2,500 to 7,000 in adults, and it is more useful than the percentage on its own, particularly when the question is whether the count is low.34 Compare both results with the interval printed on your own report, and read them alongside your symptoms, your medicines and any earlier blood counts.

If you are receiving chemotherapy or have been told you have severe neutropenia, follow your treatment team's fever instructions. A temperature of 38°C (100.4°F) or higher during the expected neutropenic period calls for urgent medical assessment. If your ANC is below 500 per microliter and you develop a fever, seek emergency care immediately.56

Neutrophils are the cells that go after bacteria first, so the count climbs with a bacterial infection, a steroid tablet, smoking or plain stress, and it falls with some viral fevers, some medicines and a few deficiencies. This page explains what the numbers on an Indian report mean, why the absolute count is the one to read, and what high and low results usually point to.

There is no single national normal range. A commonly cited adult neutrophil percentage is 40 to 60 percent, although laboratories often use wider intervals, and a typical adult absolute neutrophil count is about 2,500 to 7,000 per microliter, with neutropenia below 1,500 and neutrophilia above 7,000 or about 7,700 depending on the source.13 The upper limit depends on the source: the American Family Physician review draws the line at 7,000, StatPearls at 7,700, and your own report may print either. In infants under one year the neutropenia cut-off is 1,000, and children's percentages run lower than adults' because lymphocytes dominate in early childhood.5

Units are the same number written three ways: 4,400 /µL is 4,400 /mm³ (Indian reports write /cumm) and 4.4 ×10⁹/L. Two things stand out in that table. The Indian study's neutrophil percentages sat inside the 40 to 75 percent range its laboratories already printed, and the authors reported that the differential broadly matched reporting values; the study drew on health-check attendees between 2004 and 2008, 70 percent of them men, and its authors asked for community-based validation. It is context for a flagged result, and it does not replace the interval your laboratory chose for its own analyzer.2 And about 2.5 percent of healthy people count as neutrophilic by definition, because the upper limit is a statistical line, so an out-of-range result does not by itself establish disease.3 Check the absolute count before you read anything into your percentage. Ask for it if the report leaves it out.

What are neutrophils and what do they do? Copy link

Neutrophils are the most numerous white blood cell and the first cells recruited to a site of infection. They swallow bacteria and fungi, destroy them with reactive oxygen chemicals, and mop up injured tissue.3 Pus is mostly dead neutrophils. They are made in the bone marrow, spend about ten days maturing there, then circulate in blood for less than a day before moving into tissue, where they live two to three days.3

That last fact explains most of what confuses people about the count. A blood sample catches neutrophils in the shortest phase of their life, and only a small fraction of the body's neutrophils, about 3 to 5 percent, are in freely circulating blood at one time; large reserves sit in the marrow and in other non-circulating pools until infection or stress calls them out.4 Doubling the number in blood needs no new cells, just a release from the reserve. That is why your count moves within hours of a fever, a steroid dose or a hard run. Remember that when you compare two reports taken a day apart.

Polymorphs, GRAN, NEUT, segs and bands: what your report calls neutrophils Copy link

Polymorphs in a blood test are neutrophils. The full name is polymorphonuclear leukocytes, after the lobed nucleus, and older Indian reports print the differential as P and L for polymorphs and lymphocytes, so the polymorphs normal value is the neutrophil range above.1 The other labels are less interchangeable than they look, and the table sets out which ones mean neutrophils alone.

The abbreviations trip people up more than the cells do. NEUT, NE, polys and P all mean neutrophils, and their normal range is the neutrophil range in the first table. Segmented neutrophils are the mature cells, usually 40 to 60 percent; metamyelocytes and myelocytes are earlier stages that should read zero in blood. GRAN or GRA on a three-part analyzer is a different animal: it bundles neutrophils with eosinophils and basophils, so use the granulocyte reference interval printed by that analyzer or laboratory, and treat a high or low GRAN figure as a reason to ask for a five-part differential before anyone calls it neutrophilia or neutropenia.3

Absolute neutrophil count: the number that matters Copy link

The formula is ANC = WBC × (segmented neutrophils % + separately reported bands %) ÷ 100. A WBC of 8,000 with 55 percent neutrophils gives an ANC of 4,400. If your report folds bands into the neutrophil percentage already, use that percentage alone and do not add the bands a second time. Infection risk tracks the absolute count, and so do the treatment thresholds, which is why the percentage on its own can mislead in both directions.41

Take the two cases in the plate. A percentage of 70 with a total WBC count of 4,000 is an ANC of 2,800, comfortably normal. The same 70 percent with a total of 12,000 is an ANC of 8,400, which is neutrophilia. Reverse it and a low percentage can hide a normal count: 35 percent of 9,000 is 3,150, fine. Good labs print the absolute count for you. If yours prints only percentages, work the multiplication yourself before you worry, and ask the lab to print absolutes next time.

What does a high neutrophil count mean? Copy link

High neutrophils, called neutrophilia, most often mean a bacterial infection, inflammation or physical stress, and the count itself is a normal immune response.3 When the total WBC count is also above 11,000 the pattern is called neutrophilic leukocytosis. Neutrophils are the cell type behind most high white counts, so neutrophilic leukocytosis means what a high WBC usually means: something has called the reserve out of the marrow. As you would expect, that something is usually mundane.31

What raises your neutrophils? These are the usual causes of a high neutrophil count, roughly in the order a doctor considers them. Note where your own situation fits before you read on.

  • Bacterial infection, which can cause neutrophilia, sometimes with a left shift, toxic granulation or Döhle bodies on the smear.3
  • Inflammation without infection: rheumatoid arthritis, inflammatory bowel disease, gout flares, vasculitis.3
  • Tissue damage: surgery, burns, a heart attack. Joint replacement surgery lifts the white count by roughly 2,770 on average, with the peak on the second day, and neutrophils supply most of that lift.1
  • Medicines. Corticosteroids such as prednisolone and dexamethasone release neutrophils from the marrow within hours; lithium, adrenaline and the marrow growth factors used during chemotherapy do the same by other routes.3
  • Smoking, obesity, a missing or non-working spleen.3
  • Stress of any kind: exercise, a panic attack, a seizure, labor, even heat. These raise the count by moving cells off vessel walls; the change is often transient and may fall again after the trigger passes.3
  • Rarely, a marrow disorder such as chronic myeloid leukemia, where the count is usually very high and immature cells crowd the smear.3

Mild neutrophilic leukocytosis, a total in the low teens with neutrophils around 70 to 80 percent, is the commonest version. Reactive causes are far more common than a primary marrow disorder, but persistence and the rest of the CBC determine the work-up. A count that stays high for weeks with no infection, no steroid and no cigarette is the one that earns a smear and a look at your hemoglobin and platelets. Bring your older reports; the trend tells the doctor more than today's number.1 And a small share of high results are artefacts: platelets clumping in the tube can make an analyzer over-count neutrophils, and a repeat in a fresh tube settles it.3

What does a low neutrophil count mean? Copy link

Low neutrophils, called neutropenia, mean an absolute count below 1,500 per microliter, and the grade matters more than the fact.45 Mild neutropenia is common after some viral infections and often recovers on its own, though the time it takes varies. Severe neutropenia, below 500, means the body has almost no front line against bacteria, and any fever at that level needs emergency care immediately.5

Grades from Kim and colleagues (2025) and StatPearls (2024), references 5 and 4. Very low counts, under 100 to 200 depending on the definition used, are called agranulocytosis and are usually a drug reaction or marrow failure.74

Why would your neutrophils be low? These are the usual low neutrophil count causes. Check your medicine list against the third item; it is the one people miss.

  • A recent viral infection. Dengue, influenza, COVID and some other viral illnesses can lower neutrophils for a while; recovery time varies from person to person.18
  • Typhoid, rickettsial infection and brucellosis, which lower the count even though a bacterial infection is supposed to raise it.1
  • Medicines. Chemotherapy first, then carbimazole and methimazole for an overactive thyroid, propylthiouracil, clozapine, cotrimoxazole, sulfasalazine, dapsone, hydroxychloroquine, lamotrigine and some antibiotics. In a meta-analysis of 108 studies, clozapine was associated with neutropenia in 3.8 percent of users and severe neutropenia in 0.9 percent, mostly in the first months, which is why it requires regular blood-count monitoring according to prescribing guidance and the treating clinician.9
  • Vitamin B12, folate or copper deficiency. B12 and folate deficiency may also produce macrocytosis on the CBC.45
  • Autoimmune disease: lupus, rheumatoid arthritis, Sjögren syndrome and autoimmune thyroid disease, which can be associated with immune-mediated neutropenia.4
  • An enlarged spleen, HIV, hepatitis B or C, tuberculosis.4
  • Marrow disorders, particularly when anemia, thrombocytopenia or abnormal cells accompany the neutropenia.4

One inherited pattern deserves a mention because it fools reports. People who lack the Duffy red-cell antigen, which is common in those of African, Middle Eastern or Caribbean ancestry and uncommon in most Indian populations, can have a chronically lower circulating absolute count, sometimes below 1,500, without increased infection risk; an altered distribution of neutrophils between blood and tissue has been proposed as one explanation. For an otherwise healthy person with chronic mild neutropenia and this phenotype, guidelines advise against an extensive work-up; the phenotype is confirmed by testing, and a low count in anyone still needs a doctor to assess it.45

Neutrophils and fever: what the differential can and cannot tell you Copy link

In practice, the direction your neutrophils move during a fever is a clue, and only a clue. Dengue and some other infections can lower white blood cell counts, and India's 2023 national dengue guideline includes leukopenia in its clinical description of the illness and lists it among the common laboratory findings, alongside clinical assessment and specific diagnostic tests. Leukopenia may appear while the platelet count is still normal; a rapid platelet fall becomes particularly important around the critical phase.8 Typhoid, rickettsial fevers and brucellosis can lower neutrophils too, even though they are bacterial.1 Bacterial infections in general can raise neutrophils and put bands on the smear.3 Neither pattern identifies the infection or decides whether antibiotics are needed. Symptoms, examination, how long you have been ill and the appropriate tests do that, and a differential count on its own is insufficient.8 The Widal test has limits of its own, covered on its page.

Viral illnesses in general leave the total normal or low and hand a larger share to lymphocytes, though some can raise the count early in their course.1 When a viral illness drags on and the neutrophils stay high, a bacterial complication becomes a fair question, and the count alone cannot answer it.1 In malaria the total white cell count is usually normal or low, with 79 percent of 888 patients in one Colombian series within the normal range, 18 percent below it; the commonest change in that series was a fall in lymphocytes, and neutrophilia tracked complications. In an Indian clinic malaria is excluded with a smear or rapid test, never inferred from the differential.10

What a specific neutrophil result usually means Copy link

Look up your number below. The bands are for adults who are not pregnant; read each with the total, the smear and how you feel.

Neutrophils 75 percent, total count normal

The absolute count decides this one. With a total WBC of 7,000 the ANC is 5,250, within the commonly cited adult ranges used on this page; with a total of 10,500, also inside common WBC intervals, the same 75 percent is an ANC of 7,875, which is neutrophilia by either definition.13 The Indian reference study's upper limits were 75 percent for women and 74 for men, against a 40 to 75 percent comparator, which is context for a flagged result, and your laboratory's own interval still applies.2 A doctor reads the percentage with the absolute count, the smear and whatever symptoms brought you to the test; a high percentage with fever does not on its own establish a bacterial infection.

Neutrophils 80 percent with a WBC of 12,000

An ANC of 9,600, which is neutrophilia and, with the total above 11,000, neutrophilic leukocytosis.3 High WBC and neutrophils together are the classic bacterial pattern, but the same numbers follow a steroid dose, physical or emotional stress, a hard workout or a cigarette habit. If you can name a trigger, a repeat after it has passed is the usual step. If there is none, or the count persists, the smear and the other cell lines come next, and a doctor decides whether infection is the explanation.1

Absolute neutrophils 1,200

Mild neutropenia. After a viral infection this is common and often recovers, on a timescale that varies. If it appears on a routine test with no illness, depending on the history a clinician may consider B12, folate and copper testing, a medication review and a repeat CBC; a persistent mild count with normal hemoglobin and platelets is often idiopathic and benign.45

Absolute neutrophils 800

Moderate neutropenia. Infection risk is rising, and this merits prompt clinical review, usually including confirmation of the count, a medication review and assessment for infection or other causes: recent viral illness, a new tablet, chemotherapy, a deficiency or an autoimmune process. Any fever at this level warrants review the same day, and once the count is under 500, any fever means emergency care immediately.5

How to increase or reduce neutrophils Copy link

The short answer: you change a neutrophil count by treating whatever moved it. There is no evidence-based food or supplement that reliably treats neutropenia when no nutritional deficiency is present, and the two lifestyle causes of a high count, smoking and excess weight, take months to reverse. What does work: correcting a B12, folate or copper deficiency if one is present, a change to a medicine that is suppressing the marrow, made by the treating clinician, and letting a post-viral count recover on its own.45 Growth-factor injections, G-CSF, raise the count in severe or inherited neutropenia and in chemotherapy, and they are a specialist decision.4 On the high side, once the infection is treated or the steroid course ends, the count falls back over days.3

When to see a doctor Copy link

If you are on chemotherapy or have severe neutropenia, a temperature of 38°C (100.4°F) or higher calls for urgent medical assessment, and a fever with an ANC below 500 means emergency care immediately. The formal definition of febrile neutropenia is a single oral temperature of 38.3°C or higher, or 38°C sustained for an hour, with neutropenia, which in this setting is an ANC below 500 per microliter, or an ANC expected to fall below 500 during the next 48 hours.6 That expected fall is why the count is not something to wait for. On chemotherapy the low point usually arrives a week or two after each cycle, and the treating team assumes it. Febrile neutropenia needs immediate medical assessment. Antibiotics should begin promptly; high-risk patients need hospital treatment, while carefully selected low-risk patients may be treated as outpatients after assessment.5

Outside that situation, prompt clinical review is warranted once the ANC drops below 1,000, and sooner with fever, mouth ulcers or a worsening infection. A mild neutropenia on a routine test with no symptoms can be repeated after a few weeks once the medicine list has been checked. If you started a new tablet in the month before a low result, the doctor should reconcile every medicine you take and stop any suspect; re-exposing you to prove the point is not recommended.5

On the high side, a neutrophil count that stays raised for weeks without an infection, a steroid or a cigarette, or one that comes with night sweats, weight loss, swollen glands, or a hemoglobin or platelet count that is also off, should be reviewed with a smear rather than repeated and filed.1 A differential comes with a CBC with differential, the version of the complete blood count most Indian labs run by default, drawn with the red cell count in the same tube whether it comes alone or as part of a full body checkup. Are high neutrophils and low lymphocytes a sign of something serious?

Often they are two views of the same event. All five differential fractions add to about 100 percent, so when neutrophils rise during a bacterial infection or after a steroid dose, the lymphocyte percentage falls. Read the absolute lymphocyte count; if it is normal, the low percentage is arithmetic, and a doctor decides what the neutrophil rise means.

Can a viral fever cause low neutrophils?

Yes. Some viral infections, including dengue, influenza and COVID, can cause a temporary fall in neutrophils, and recovery time varies. Typhoid can lower neutrophils as well; in malaria the total white cell count is usually normal or low, and lymphocytes fall more often than neutrophils. A repeat after recovery, and a doctor's review if it stays low, is the usual course.

What neutrophil count is dangerous?

Once the absolute count is under 500 per microliter, any fever means emergency care immediately, because serious bacterial infection can develop within hours. If you are on chemotherapy or have severe neutropenia, a temperature of 38°C or higher calls for urgent medical assessment. Below 1,000 needs prompt review. On the high side there is no single dangerous number; very high counts with immature cells on the smear need a hematologist.

Do I need to fast for a neutrophil test?

No. The differential is part of a CBC with differential, which can be drawn at any time of day, fed or fasting. Fasting only matters when a glucose or lipid test shares the same visit.
